Facility and resident characteristics associated with variation in nursing home transfers: evidence from the OPTIMISTIC demonstration project

Abstract Background Centers for Medicare and Medicaid Services (CMS) funded demonstration project to evaluate financial incentives nursing facilities providing care 6 clinical conditions reduce potentially avoidable hospitalizations (PAHs). The Optimizing Patient Transfers, Impacting Medical Quality, Improving Symptoms: Transforming Institutional Care (OPTIMISTIC) site tested payment alone in combination with the successful nurse-led OPTIMISTIC model. Our objective was identify facility resident characteristics associated transfers, including or without Methods This a longitudinal analysis from April 2017 June 2018 of transfers among home residents 40 facilities, 17 had full + model (1726 residents) 23 only (2142 residents). Using CMS claims data, Minimum Data Set, Nursing Home Compare, multilevel logit models estimated likelihood all-cause PAHs (based on data ICD-codes) characteristics. Results 4.1 percentage points (pps) lower risk (95% confidence interval [CI] ? 6.2 2.1). Characteristics PAH included aged 95+ years (? 2.4 pps; 95% CI 3.8 1.1), Medicare-Medicaid dual-eligibility 2.5 3.3 1.7), advanced moderate cognitive impairment 4.4 2.1; 1.2 2.2 0.2). Changes Health, End-stage disease Symptoms Signs (CHESS) score above most stable (CHESS 4) increased by 7.3 pps 1.5 13.1). Conclusions Multiple are transfers. Facilities demonstrated compared those only, but not PAHs.
